ABSTRACT

Objetivo:Classificar o risco de desenvolvimento de lesão por posicionamento cirúrgico. Método: Estudo observacional, longitudinal, prospectivo, de abordagem quantitativa realizado em hospital público, com 135 pacientes submetidos à cirurgia eletiva. Utilizaram-se instrumentos contendo caracterização sociodemográfica, clínica e cirúrgica e escala de avaliação de risco para desenvolvimento de lesões decorrentes do posicionamento cirúrgico. Empregaram-se análise descritiva, teste exato de Fisher ou teste χ2 e a medida de associação odds ratio, conforme apropriado. Resultados: A maioria dos participantes era do sexo masculino (51,11%), adulta (52,59%) e foi classificada como maior risco para o desenvolvimento de lesões por posicionamento cirúrgico (51,85%). Ser idoso, hipertensão, diabetes mellitus e cirurgias urológicas foram estatisticamente significativos (p < 0,05) para maior risco de desenvolvimento de lesões. A incidência de lesão por pressão foi de 0,74%, com observação apenas na região sacra. Conclusão: Verificou-se maior risco para desenvolvimento de lesão em decorrência do posicionamento cirúrgico e baixa incidência de lesão por pressão. A enfermagem perioperatória deve incorporar à prática assistencial ferramentas validadas de mensuração de risco para um cuidado seguro, individualizado e de qualidade aos pacientes cirúrgicos.


Objective:To classify the risk of developing injury due to surgical positioning. Method: Observational, longitudinal, prospective study with a quantitative approach carried out in a public hospital, with 135 patients undergoing elective surgery. Instruments containing sociodemographic, clinical, and surgical characteristics and a risk assessment scale for the development of injuries due to surgical positioning were used. Descriptive analysis, Fisher's exact test or χ2 test and odds ratio association measure were used as appropriate. Results: Most participants were male (51.11%), adults (52.59%) and were classified as having a higher risk for developing injuries due to surgical positioning (51.85%). Elderly, hypertension, diabetes mellitus and urological surgeries were statistically significant (p < 0.05) for a higher risk of developing lesions. The incidence of pressure injuries was 0.74%, with observation only in the sacral region. Conclusion: There was a greater risk of developing lesions due to surgical positioning and low incidence of pressure injury. Perioperative nursing should incorporate validated risk measurement tools into care practice for safe, individualized and quality care for surgical patients,


Objetivo:Clasificar el riesgo de desarrollar lesión por posicionamiento quirúrgico. Método: Estudio observacional, longitudinal, prospectivo, con abordaje cuantitativo, realizado en un hospital público, con 135 pacientes sometidos a cirugía electiva. Se utilizaron instrumentos que contenían características sociodemográficas, clínicas y quirúrgicas y una Escala de Evaluación de Riesgo para el Desarrollo de Lesiones por Posicionamiento Quirúrgico. Se utilizó el análisis descriptivo, la prueba exacta de Fisher, o chi-cuadrado y la medida de asociación odds ratio, según corresponda. Resultados: La mayoría de los participantes eran hombres (51,11 %), adultos (52,59 %) y se clasificaron con mayor riesgo de desarrollar lesiones debido al posicionamiento quirúrgico (51,85 %). Ancianos, hipertensión, diabetes mellitus y cirugías urológicas fueron estadísticamente significativos (p ˂ 0,05) para mayor riesgo de desarrollar lesiones. La incidencia de lesiones por presión fue del 0,74%, observándose solo en la región sacra. Conclusión: Hubo un mayor riesgo de desarrollar lesiones debido al posicionamiento quirúrgico y una baja incidencia de lesión presión. La enfermería perioperatoria debe incorporar herramientas validadas de medición del riesgo en la práctica asistencial para una atención segura, individualizada y de calidad a los pacientes quirúrgicos.

SUMMARY: Skeletal muscles play a fundamental role in people's lives and their evaluation provides significant information on health. Different tools have been used to evaluate muscle mass, and the evaluation of muscle thickness (MT) using ultrasound has been included as an alternative, which can be performed with the probe in different positions; however, these could present differences. The objectives of this study were to determine whether there are differences in the measurement of MT in the vastus lateralis (VL) muscle using the probe in the longitudinal or transverse position, and to determine its association with the lean mass of the lower limbs. The results indicated no significant differences between MT measurements with the probe in the longitudinal and transverse positions (p =0.084). However, when associating these measurements with lower limb lean mass, it was found that transverse measurements had a strong association (r =0.547; p < 0.001), while longitudinal measurements had a moderate association (r =0.351; p =0.007). This suggests that measurements with the probe positioned transversely to measure the MT would be the best option. Therefore, it could be useful as an indicator of lower limb lean mass in the absence of tools, such as bioelectrical bioimpedance or magnetic nuclear resonance.


El músculo esquelético cumple un rol fundamental en la vida de las personas, y su evaluación entrega mucha información de la salud. Se han utilizado diferentes herramientas para evaluar la masa muscular, y el último tiempo se ha incluido la evaluación del grosor muscular (MT) a través de la ecografía como una alternativa para estimarla, las cuales se pueden realizar con la sonda en distintas posiciones, sin embargo, estas podrían presentar diferencias. Los objetivos del estudio fueron determinar si existen diferencias en la medición de MT en el músculo vasto lateral (VL) utilizando la sonda en posición longitudinal o transversal y determinar su asociación con la masa magra de los miembros inferiores. Los resultados indican que no existen diferencias significativas entre las mediciones de MT con la sonda en posición longitudinal y transversal (valor p: 0.084). Sin embargo, al asociar estas mediciones con la masa magra de los miembros inferiores, se encontró que las mediciones transversales poseen una asociación fuerte (r: 0.547; valor p < 0.001), mientras que las mediciones longitudinales presentan una asociación moderada (r: 0.351; valor p: 0.007). Esto sugiere que las mediciones con la sonda posicionada transversal para medir MT serían la mejor opción. Por lo tanto, podría ser de utilidad como un indicador de masa magra de los miembros inferiores en caso de no contar con herramientas como la bioimpedancia bioeléctrica o resonancia nuclear magnética.

ABSTRACT

The patient's correct position is necessary to conduct a safe and effective surgical procedure. This position depends on the access route, duration of the procedure, kind of anesthesia, devices to use, among other factors. This procedure requires planning and effort by the surgical team where they share responsibility to establish and maintain the correct positions for patients. Each surgical position fulfills an objective and implies risks to patients, which is why nursing professionals must be very attentive to provide the necessary care and ensure reliable practices in each position during the perioperative, the importance of the documentation, and the NANDA, NIC, and NOC taxonomy to consider.


La correcta posición del paciente es necesaria para el desarrollo de un procedimiento quirúrgico seguro y eficaz. Esta posición depende de la vía de acceso, la duración del procedimiento, el tipo de anestesia, los dispositivos a utilizar entre otros factores. Este procedimiento requiere planeación y esfuerzo del equipo quirúrgico donde se comparten responsabilidad para establecer y mantener las posiciones correctas para los pacientes. Cada posición quirúrgica cumple un objetivo e implica un riesgo para el paciente, por lo que los profesionales de enfermería deben estar muy atentos para brindar los cuidados necesarios y asegurar prácticas confiables en cada posición durante el perioperatorio, la importancia de la documentación y la taxonomía NANDA, NIC y NOC a tener en cuenta.


A posição correta do paciente é necessária para o desenvolvimento de um procedimento cirúrgico seguro e eficaz. Essa posição depende da via de acesso, da duração do procedimento, do tipo de anestesia, dos aparelhos a serem utilizados, entre outros fatores. Este procedimento requer planejamento e esforço da equipe cirúrgica onde a responsabilidade é compartilhada para estabelecer e manter as posições corretas dos pacientes. Cada posição cirúrgica cumpre um objetivo e implica um risco para o paciente, por isso os profissionais de enfermagem devem estar muito atentos para prestar os cuidados necessários e garantir práticas confiáveis em cada posição durante o período perioperatório, a importância da documentação e taxonomia NANDA, NIC e NOC para levar em conta.

ABSTRACT

Objective:To evaluate the influence of an additional roll test on the repositioning procedure by SRM-vertigo diagnosis system for horizontal canal benign paroxysmal positional vertigo(HC-BPPV). Methods:A total of 713 patients diagnosed with HC-BPPV in Department of Otolaryngology Head and Neck Surgery,the First Affiliated Hospital of Xi'an Jiaotong University from Jan 2020 to Feb 2022 were enrolled. The patients were divided into two groups by hospital card numbers, in which the number is odd were considered as group A, and the number is even were considered as group B. The group A underwent two circles of Barbecue repositioning procedure by SRM-vertigo diagnosis system, while the group B first performed an additional roll test and then underwent two circles of Barbecue repositioning procedure by SRM-vertigo diagnosis system, to observe the cure rate and compare influence of HC-BPPV by an additional roll test. The quality of life and sleep of patients before and one-month after the treatment were assessed by the dizziness handicap inventory(DHI) and the pittsburgh sleep quality(PSQI). Results:The cure rate of group A was 63.21%, and the cure rate of group B was 87.68%,the difference between the two groups was statistically significant(P<0.05); The DHI score of patients after the repositioning was significantly lower than that before the repositioning(P<0.05). The PSQI score after the repositioning was significantly lower than that before the repositioning(P<0.05). The DHI and the PSQI scores after the repositioning were significantly lower than that before the repositioning, with a statistically significant difference (P< 0.05). The total score of DHI in group B after treatment was lower than that in group A, with a statistically significant difference(P<0.05). The total score of PSQI in group B after treatment was lower than that in group A, with non-statistically significant difference (P< 0.05). Conclusion:An additional roll test before the repositioning procedure by SRM-vertigo diagnosis system can significantly improve the cure rate of HC-BPPV, relieve anxiety, and improve the quality of life.

Abstract Spinal cord infarction is an uncommon phenomenon, which can be caused by different etiologies, constituting a real diagnostic challenge which can lead to devastating consequences. General anesthesia in beach chair positioning with intraoperative hypotension in order to avoid surgical bleeding are associated with hypoperfusion and potential neurological ischemia-related complications. We present a case of spinal cord ischemia in the context of shoulder surgery in a beach chair position.

Abstract Introduction Multiple studies have shown the effects of prone (PP), supine (SP) and kangaroo (KP) positions on clinical and physiological outcomes in preterm newborns, but none compared these three types of positioning between them. Objective To investigate the influence of these positionings on heart rate, respiratory rate, peripheral oxygen saturation (SpO2) and alertness status in clinically stable preterm newborns (NBs) admitted to a neonatal intensive care unit. Methods In a randomized clinical trial, clinically stable NBs with gestational ages from 30 to 37 weeks who were breathing spontaneously were allocated in three positioning groups: PP, SP and KP. Heart rate, breathing frequency, SpO2 and alertness status were evaluated immediately before and after 30 minutes of positioning. Results In all, 66 NBs were assessed (corrected age: 35.48 ± 1.94 weeks; weight: 1840.14 ± 361.09 g), (PP: n = 22; SP: n = 23; KP: n = 21). NBs in the PP group showed a significant improvement in peripheral SpO2 (97.18 ± 2.16 vs 95.47 ± 2.93 vs 95.57 ± 2.95, p = 0.03) compared with the SP and KP groups. Conclusion In clinically stable preterm NBs, the PP was associated with better peripheral oxygen saturation than the SP or KP. In addition, there was a reduction in heart rate within prone position group and in the KP group there was an increase in the number of NBs in the deep sleep classification.


Resumo Introdução Vários estudos têm demonstrado os efeitos das posições prona (PP), supina (SP) e canguru (KP) sobre os resul-tados clínicos e fisiológicos em recém-nascidos prematuros, mas nenhum comparou esses três tipos de posicionamento. Objetivo Investigar a influência desses posicionamentos na frequência cardíaca, frequência respiratória, saturação periférica de oxigênio (SpO2) e estado de alerta em recém-nascidos pré-termo (RN) clinicamente estáveis internados em uma unidade de terapia intensiva neonatal. Métodos Em um ensaio clínico randomizado, RN clinicamente estáveis com idade gestacional de 30 a 37 semanas e respirando espontaneamente foram alocados em três grupos de posicionamento: PP, SP e KP. Frequência cardíaca e respiratória, SpO2 e estado de alerta foram avaliados imediatamente antes e após 30 minutos de posicionamento. Resultados Ao todo, foram avaliados 66 RNs (idade corrigida: 35,48 ± 1,94 semanas; peso: 1840,14 ± 361,09 g), (PP: n = 22; SP: n = 23; KP: n = 21). Os RNs do grupo PP apresentaram melhora significativa na SpO2 periférica (97,18 ± 2,16 vs 95,47 ± 2,93 vs 95,57 ± 2,95, p = 0,03) em comparação aos grupos SP e KP. Conclusão Em RN prematuros clinicamente estáveis, o PP foi associado à melhor saturação periférica de oxigênio do que o SP ou KP. Além disso, houve redução da frequência cardíaca no grupo de posição prona e no grupo KP houve aumento do número de RNs na classificação sono profundo.

ABSTRACT

Objetivo: identificar quais os posicionamentos e produtos auxiliares mais indicados para o recém-nascido hospitalizado em Unidade de Terapia Intensiva Neonatal. Método: caracteriza-se como básica, de caráter exploratório e quanto aos procedimentos técnicos é uma revisão sistemática da literatura. A busca teve como bases a Biblioteca Virtual em Saúde, Plataforma Capes e Unique, limitando as publicações realizadas entre 2011 e 2021. Obteve-se 85 resultados, sendo a análise e discussão dos dados realizada com 7 estudos. Resultados: verificou-se que existem diversos posicionamentos que podem ser utilizados nesses pacientes, como os decúbitos dorsal, ventral e lateral, sendo o ventral o mais indicado. Ademais, identificaram-se diversos produtos que podem ser utilizados para auxiliar no posicionamento dos recém-nascidos e que podem facilitar os cuidados de enfermagem. Considerações finais: observou-se que poucos estudos trouxeram um padrão e/ou um método de posicionamento vinculado com os produtos auxiliares.


Objective: to identify which are the most suitable positioning and auxiliary products for the newborn hospitalized in Neonatal Intensive Care Units. Method: this is characterized as basic, exploratory, and as to the technical procedures, it is a systematic literature review. The search was based on the Biblioteca Virtual em Saúde, Plataforma Capes and Unique, limiting publications between 2011 and 2021. A total of 85 results were obtained, and the analysis and discussion of the data was carried out with 7 studies. Results: it was found that there are several positions that can be used in these patients, such as dorsal, ventral, and lateral decubitus, with the ventral position being the most indicated. Furthermore, several products were identified that can be used to assist in positioning the newborns and that can facilitate nursing care. Final considerations: it was observed that few studies brought a standard and/or a positioning method linked to the auxiliary products.


Objetivo: identificar cuáles son los posicionamientos y productos auxiliares más adecuados para los recién nacidos hospitalizados en una Unidad de Cuidados Intensivos Neonatales. Método: se caracteriza como básico, de carácter exploratorio y en cuanto a los procedimientos técnicos es una revisión sistemática de la literatura. La búsqueda se basó en la Biblioteca Virtual em Saúde, Plataforma Capes y Unique, limitando las publicaciones entre 2011 y 2021. Se obtuvieron 85 resultados, siendo el análisis y la discusión de los datos realizados con 7 estudios. Resultados: Se ha comprobado que hay varias posiciones que pueden utilizarse en estos pacientes, como el decúbito dorsal, ventral y lateral, siendo la ventral la más adecuada. Además, se identificaron varios productos que pueden utilizarse para ayudar a colocar a los recién nacidos y que pueden facilitar los cuidados de enfermería. Consideraciones finales: se observó que pocos estudios trajeron un patrón y/o un método de posicionamiento vinculado a los productos auxiliares.

Abstract Background and objectives The Beach Chair Position (BCP) has many advantages such as less neurovascular injury and better intra-articular visualization, but it has also negative consequences, including hemodynamic instability. Although maintaining normal Mean Arterial Pressure (MAP) is important, fluid management is also a crucial concept for hemodynamic stability. The main objective of this study is whether preloading before positioning would be effective for less hemodynamic instability. Methods This randomized, controlled study was conducted in a single center in the Istanbul University, Istanbul Faculty of Medicine. Forty-nine patients undergoing elective arthroscopic surgery in the BCP were recruited. In the study group, crystalloid fluid at 10 mL.kg-1 of ideal body weight was administered intravenously 30 min before the BCP for preloading. The primary outcome measures were differences of hemodynamic variables as MAP, Stroke Volume (SV), Heart Rate (HR), and Cardiac Output (CO). The secondary outcome measures were Postoperative Nausea and Vomiting (PONV) rates in postoperative first day, surgical satisfaction scale, total ephedrine dose used during surgery, and total amount of fluid. Results The MAP, CO, and SV measurements of the study group were higher than those of the control group in the 5th minute after the BCP (respectively, p= 0.001, p= 0.016, p= 0.01). The total amount of crystalloid and surgical satisfaction scales were higher in the study group (respectively, p= 0.016, p= 0.001). Total amount of colloid and ephedrine dose used in the intraoperative period, and PONV rates were lower in the study group (p= 0.003, p= 0.018, p= 0.019, respectively). Conclusion Consequently, preloading can be favorable approach to preserve hemodynamic stability.

Objetivo: relacionar o escore de risco de lesões por posicionamento cirúrgico decorrentes da posição supina com aspectos sociodemográficos, clínicos, cirúrgicos e ocorrência de complicações. Método: estudo observacional longitudinal realizado com 89 pacientes em decúbito dorsal. Aplicadas variáveis ​​sociodemográficas e clínicas e a Escala de Risco para Desenvolvimento de Lesões por Posicionamento Cirúrgico. Adotadas análises descritivas, bivariadas e de regressão logística, considerando-se um nível de significância de α=0,05. Resultados: a idade aumentou em 1,11 vezes (p<0,001) e a obesidade em 13,77 vezes (p=0,01) a chance de aumento do risco de lesões. A proporção de dor (34,1%) e lesão por pressão na região sacrococcígea (91,7%) destacou-se nos pacientes de maior risco (p=0,05). Conclusão: obesos e idosos apresentaram maior risco de lesões. Dor e ocorrência de lesão por pressão na região sacrococcígea foram as complicações predominantes nos pacientes de maior risco (AU)

Método:estudio observacional longitudinal realizado con 89 pacientes en decúbito supino. Se aplicaron variables sociodemográficas, clínicas y la Escala de Riesgo para el Desarrollo de Lesiones de Posicionamiento Quirúrgico. Fueron adoptados análisis descriptivos, bivariados y de regresión logística, considerando un nivel de significancia de α=0,05. Resultados:La edad aumentó en 1,11 veces (p<0,001) y la obesidad en 13,77 veces (p=0,01) la probabilidad de mayor riesgo de lesión. La proporción de dolor (34,1%) y lesión por presión en la región sacrococcígea (91,7%) sedestacó en los pacientes de mayor riesgo (p=0,05).Conclusión:Las personas obesas y ancianas tenían mayor riesgo de lesiones. El dolor y la aparición de lesión por presión en la región sacrococcígea fueron las complicaciones predominantes en los pacientes de mayor riesgo (AU)

Resumen Se han comunicado buenos resultados clínicos al poner en posición prono a pacientes con Síndrome de Distrés Respiratorio Agudo por COVID-19. Objetivo: Describir la maniobra prono, sus resultados clínicos y cuidados asociados, en una mujer de 34 años de edad con 26 semanas de embarazo, que estaba en ventilación mecánica, por un cuadro clínico de neumonía multifocal por COVID-19. Se realizó maniobra prono lateralizada hacia izquierda, en tres etapas, preparación, ejecución y evaluación. Luego de 62 h de prono, se observó una recuperación favorable de la gestante: la relación PaO2/FiO2 aumentó de 151 a 368 mmHg, y disminuyó el compromiso radiológico pulmonar, sin que se detectaran complicaciones fetales. Conclusiones: Esta maniobra que puede beneficiar a pacientes con insuficiencia respiratoria grave, en embarazadas debe ser una técnica protocolizada, con equipos de trabajo experimentados e implementación adecuada.


Good clinical results have been reported when placing patients with acute respiratory distress syndrome due to COVID-19 in a prone position. Objective: To describe the prone maneuver, its clinical results and associated care in a 34-year-old woman with 26 weeks of pregnancy, who was on mechanical ventilation, due to a clinical picture of COVID-19 multifocal pneumonia. Lateralized prone maneuver was carried out to the left, in three stages, preparation, execution and evaluation. After 62 hours of prone, a favorable recovery of the pregnant woman was observed: PaO2/FiO2 ratio increased from 151 to 368 mmHg, and the pulmonary radiological compromise decreased, without fetal complications being detected. Conclusions: This maneuver that might benefit patients with severe respiratory failure, in pregnant women should be a protocolized technique, with experienced work teams and adequate implementation.

Resumen Introducción: El vértigo posicional paroxístico benigno (VPPB) es la afección periférica más común en las enfermedades otoneurológicas. Con el reposicionamiento de partículas se busca eliminar el vértigo y sus síntomas asociados como lo son el mareo residual y la inestabilidad. Objetivo: Determinar si la maniobra de reposicionamiento de Epley (MRE) produce una modificación significativa del control postural (CP) en aquellos pacientes con VPPB de canal semicircular posterior (VPPB-CSC-P). Material y Método: Se realizó un estudio descriptivo prospectivo en una muestra de 21 pacientes con diagnóstico de VPPB-CSC-P. Comparamos el desplazamiento, la velocidad y el área del centro de presión (CoP) antes y después de la MRE. Resultados: La velocidad y el área de la CoP estudiada por posturografía computarizada muestra una disminución significativa en sus valores después de la MRE, mientras que el desplazamiento de la CoP se mantuvo sin cambios. Conclusión: La MRE ejecutada en pacientes con VPPB-CSC-P produce una modulación en el control de la CoP, demostrada por la disminución de la velocidad y el área de desplazamiento de la CoP. El éxito de la MRE produce modulación del CP.


Abstract Introduction: Benign paroxysmal positional vertigo (BPPV) is the most common peripheral condition in otoneurologic diseases. With the repositioning of particles, the aim is to eliminate vertigo and its associated symptoms, such as residual dizziness and instability. Aim: To determine if the Epley repositioning maneuver (ERM) produces a significant modification of postural control (PC) in those patients with posterior semicircular canal BPPV (BPPV-CSC-P). Material and Method: A prospective descriptive study was carried out in a sample of 21 patients diagnosed with BPPV-CSC-P. We compared the displacement, velocity, and area of the center of pressure (CoP) before and after the Epley repositioning maneuver. Results: The velocity and the area of the CoP studied by computed posturography show a significant decrease in its values after the MRE, while the CoP shift remained unchanged. Conclusion: ERM performed in patients with BPPV-CSC-P produces an improvement in the control of the CoP, demonstrated by the decrease in the speed and the area of movement of the CoP. The success of the MRE produces modulation of the PC.

ABSTRACT

Objetivo: Analisar se boas práticas de atenção ao parto estão sendo executadas e quais necessitam ser aperfeiçoadas no Hospital Materno Infantil Nossa Senhora de Nazaré (HMINSN), em Boa Vista-RR. Métodos: Foram registrados os partos normais de setembro de 2019 a março de 2020, pela equipe administrativa do hospital, na base de dados do Apice On. Nove recomendações de boas práticas da Organização Mundial da Saúde (OMS) foram avaliadas e comparadas com outros serviços do País. Resultados: Observou-se que, em comparação com outros serviços do Brasil, entre as nove práticas analisadas, quatro apresentaram melhor resultado no HMINSN, com alta proporção de acompanhantes (90,1%), aplicação de ocitocina no terceiro período (98,7%), amamentação na primeira hora pós-parto (81,5%) e baixo número de episiotomia (8,8%). Além disso, atingiu metas do Apice On em quatro boas práticas, que são a presença de acompanhante (meta: acima de 90%), a aplicação de ocitocina no terceiro período (meta: acima de 90%), o clampeamento tardio do cordão umbilical (meta: acima de 90%) e a episiotomia (meta: abaixo de 10%). Conclusão: Este estudo identificou que é preciso melhorar as taxas de prescrição de dieta livre e o contato pele a pele na primeira hora pós-parto. De modo geral, o HMINSN tem apresentado bons indicadores em relação a outros serviços do País e vem buscando aprimorar a organização da equipe e do serviço para que todas as recomendações da OMS sejam efetivamente praticadas.(AU)


Objective: Analyze whether good practices for child care are being carried out and which need to be improved at the Hospital Materno Infantil Nossa Senhora de Nazaré (HMINSN), in Boa Vista-RR. Methods: Normal deliveries from September 2019 to March 2020 were recorded by the hospital's administrative staff in the Apice On database. Nine recommendations of World Health Organization (WHO) good practices were evaluated and compared with other services in the country. Results: It was observed, in comparison with other services in Brazil, among the nine practices analyzed, four of them had better results at HMINSN, with a high proportion of companions (90.1%), application of oxytocin in the third period (98.7 %), breastfeeding in the first hour postpartum (81.5%) and low number of episiotomy (8.8%). In addition, it reached Apice On targets in four good practices, which are the presence of a companion (target: above 90%), the application of oxytocin in the third period (target: above 90%), the late clamping of the umbilical cord (target: above 90%) and episiotomy (target: below 10%). Conclusion: This study identified that it is necessary to improve the prescription rates of free diet and skin-to-skin contact, in the first postpartum hour. In general, HMINSN has presented good indicators in relation to other services in the country and has been seeking to improve the organization of the team and the service, so that all WHO recommendations are effectively practiced.(AU)

Objetivo: avaliar o risco para desenvolvimento de lesões relacionadas ao posicionamento cirúrgico. Método: estudo transversal com amostra (n=146) de adultos cirúrgicos eletivos do Paraná, Brasil. Aplicou-se a Escala de Avaliação de Risco para o Desenvolvimento de Lesões decorrentes do Posicionamento Cirúrgico e extração de variáveis sociodemográficas. Procedeu-se análise estatística descritiva e inferencial. Resultados: homens (49,3%) e mulheres (50,7%) tiveram distribuição equânime de atendimento majoritariamente pela ortopedia (39,7%). Pela escala aplicada, destacou-se: uso de colchão convencional+coxins de campo de algodão como superfície de suporte (100%); posição supina (78%); membros superiores com abertura inferior a 90º (70,5%); anestesia regional (54,1%) e tempo cirúrgico de 1-2h (46,6%). Constatou-se escore médio de 17,6±3,29 pontos e 75,3% da amostra apresentou baixo risco de desenvolver lesões do posicionamento cirúrgico. Conclusão: em torno de um quarto da amostra apresentou maior risco de lesão decorrente da posição cirúrgica.


Objetivo: evaluar el riesgo de desarrollar lesiones relacionadas con la posición quirúrgica. Método: estudio transversal con una muestra (n=146) de adultos sometidos a cirugías electivas en Paraná, Brasil. Se aplicó la Escala de Evaluación del Riesgo de Desarrollar Lesiones a raíz de la Posición Quirúrgica y se extrajeron variables sociodemográficas. Se realizó un análisis estadístico descriptivo e inferencial. Resultados: hubo distribución igualitaria entre hombres (49,3%) y mujeres (50,7%), atendidos mayoritariamente en la especialidad de Ortopedia (39,7%). Al aplicar la escala se destacó lo siguiente: utilización de colchón convencional + almohadones de campo de algodón como superficie de apoyo (100%); posición supina (78%); extremidades superiores con abertura de menos de 90º (70,5%); anestesia regional (54,1%) y duración de la cirugía de 1 a 2 horas (46,6%). Se verificó una puntuación media de 17,6±3,29 puntos y el 75,3% de la muestra presentó bajo riesgo de desarrollar lesiones a raíz de la posición quirúrgica. Conclusión: aproximadamente el 25% de la muestra presentó un riesgo más elevado de lesiones resultantes de la posición quirúrgica.


Objective: to assess the risk for the development of injuries related to surgical positioning. Method: a cross-sectional study with a sample comprised by adults undergoing elective surgeries (n=146) in Paraná, Brazil. The Risk Assessment Scale for the Development of Injuries arising from Surgical Positioning was applied, with extraction of sociodemographic variables. Descriptive and inferential analyses were performed. Results: men (49.3%) and women (50.7%) presented equal distribution of appointments, mostly in the Orthopedics specialty (39.7%). The following stood out as per the scale applied: use of conventional mattress + cotton field cushions as a support surface (100%); supine position (78%); upper limbs with opening less than 90º (70.5%); regional anesthesia (54.1%), and surgical time of 1h-2h (46.6%). A mean score of 17.6±3.29 points was found and 75.3% of the sample presented low risk for the development of injuries related to surgical positioning. Conclusion: around 25% of the sample presented a higher risk of injuries arising from surgical positioning.

ABSTRACT

Abstract This patient was a 73-year-old man who initially came to our service with acute respiratory failure secondary to COVID-19. Soon after hospitalization, he was submitted to orotracheal intubation and placed in the prone position to improve hypoxia, due to severe acute respiratory syndrome (SARS). On the third day of hospitalization, he developed acute oliguric kidney injury and volume overload. The nephrology service was activated to obtain deep venous access for renal replacement therapy (RRT). The patient could not be placed in the supine position due to significant hypoxemia. A 50-cm Permcath (MAHURKARTM, Covidien, Massachusetts, USA) was inserted through the left popliteal vein. This case report describes a possible challenging scenario that the interventional nephrologist may encounter when dealing with patients with COVID-19 with respiratory impairment in the prone position.


Resumo O paciente era um homem de 73 anos de idade que inicialmente veio ao nosso serviço com insuficiência respiratória aguda secundária à COVID-19. Logo após a internação, ele foi submetido à intubação orotraqueal e pronado para melhorar a hipóxia devido à síndrome respiratória aguda grave (SARS - do inglês "severe acute respiratory syndrome"). No terceiro dia de internação, o mesmo desenvolveu lesão renal aguda oligúrica e sobrecarga de volume. O serviço de nefrologia foi acionado para realizar acesso venoso profundo para terapia renal substitutiva (TRS). O paciente não pôde ser colocado na posição de decúbito dorsal devido a uma hipoxemia significativa. Um Permcath de 50 cm (MAHURKARTM, Covidien, Massachusetts, EUA) foi inserido através da veia poplítea esquerda. Este relato de caso descreve um possível cenário desafiador com o qual o nefrologista intervencionista pode se deparar ao lidar com pacientes com COVID-19 com problemas respiratórios e colocados em pronação.

ABSTRACT Objective: To evaluate the importance of radiography in the orthostatic position in the initial assessment of patients with thoracolumbar transition fractures and whether this image changes the surgical indication. Methods: Medical records and imaging tests of patients treated for thoracolumbar transition fractures from June 2018 to June 2019 were evaluated. Trauma patients between 18 and 60 years of age with fractures of T10 to L3 who had been indicated for conservative treatment were included. Cases of fractures considered unstable were excluded. Radiographs were taken with the patient in the supine position (supine X-ray), computed tomography (CT), and orthostatic radiography (orthostatic X-ray). Segmental kyphosis and degree of wedging were evaluated. The measurements were compared using the Wilcoxon test. The McNemar test was used to assess changes in conduct according to the criteria for surgical indication (kyphosis ≥ 25 ° and wedging ≥ 50%). Results: Fifty patients were evaluated, nine of whom (18%) were indicated for a change of conduct according to the orthostatic examinations and were submitted to surgical treatment. Vertebral kyphosis increased by 40.6% (p <0.001). The wedging increased by 25.62% (p <0.0001). Conclusion: Eighteen percent of the total number of patients who did not present instability criteria in radiographs in the supine position satisfied at least one of these criteria when the orthostatic X-ray was performed. Métodos: Foram avaliados prontuários e exames de imagens dos pacientes atendidos com fraturas da transição toracolombar, no período de junho 2018 a junho 2019. Foram incluídos pacientes vítimas de trauma, entre 18 e 60 anos de idade, com fraturas de T10 a L3, que tinham indicação de tratamento conservador. Foram excluídos os casos de fraturas consideradas instáveis já na avaliação inicial. Foram realizadas radiografias com o paciente na posição supina (Rx supino), tomografia computadorizada (TC) e radiografia ortostática (Rx ortostático). Foram avaliados a cifose segmentar e o grau de cunha. As medidas foram comparadas com o teste de Wilcoxon. Foi usado o teste de McNemar para avaliar mudanças de conduta de acordo com os critérios de indicação cirúrgica (cifose ≥ 25° e cunha ≥ 50%). Resultados: Foram avaliados 50 pacientes, sendo que nove (18%) tiveram indicação de mudança de conduta de acordo com os exames ortostáticos e foram submetidos a tratamento cirúrgico. A cifose vertebral aumentou 40,6 % (p < 0,001). O grau da cunha aumentou 25,62% (p < 0,0001). Conclusões: Do total, 18% dos pacientes que não apresentavam critérios de instabilidade nas radiografias em posição supina apresentaram pelo menos um desses critérios quando se realizou o Rx ortostático. Objective: We conducted a Meta-analysis to investigate the necessity of postural restrictions after manual reduction in the treatment of posterior canal benign paroxysmal positional vertigo (PC-BPPV). Methods: We searched PubMed, EBSCO, Proquest, Web of Science databases, Ovid, and screened eligible studies that investigated the effect of post-maneuver postural restriction in treating patients with PC-BPPV. Outcomes included the efficacy of treatment and recurrence. Meta-analysis was performed using Stata 15.0 software. Results: Studies of the single visit efficiency included 11 references, with a sample size of 1 733 cases. The Meta-analysis results showed that the difference in the efficacy between the postural restricted group and the non-postural restricted group in PC-BPPV patients was statistically significant(RR=1.12, 95%CI=1.07-1.18, P<0.001). There were 12 references included in the study on the total efficiency, with a cumulative sample size of 1763 cases. There was no statistically significant difference between the effect of postural restriction after manipulative reduction and that of simple manipulative reduction (RR=1.03, 95%CI=0.99-1.08, P=0.118). There were 5 references included in the study of recurrence rate, and the cumulative sample size was 659 cases. There was no statistically significant difference in the recurrence rate between the postural restricted group and the non-postural restricted group(RR=0.98, 95%CI=0.62-1.54, P=0.937). Conclusions: In comparison with non-postural restriction group, post-maneuver postural restriction after a single visit can improve the treatment effective rate of PC-BPPV and contribute to the improvement in the symptoms of patients in a short term. However, postural restrictions has no significant effect on the final prognosis of PC-BPPV, and it also has no significant effect on the recurrence.

Objective: To compare the effects of different intervention strategies for the management of residual dizziness following successful canalith repositioning procedure (CRP) in patients with benign paroxysmal positional vertigo (BPPV). Methods: A total of 129 BPPV patients with residual dizziness following successful CRP were recruited during January 2019 and July 2019. They were randomly assigned into three groups with 43 cases in each group: the vestibular rehabilitation group received rehabilitation training for four weeks; betahistine group was given orally 12 mg betahistine three times a day for four weeks; and the control group had no specific treatment. The primary outcomes were daily activities and social participation assessed by the Vestibular Activities and Participation measure (VAP). Secondary outcomes includedbalance function assessed by sensory organization test (SOT) and the duration of residual symptoms. Stata15.0 software was used for statistical analysis. Results: The scores of VAP in the three groups decreased over time, but a more significant decrease was found in vestibular rehabilitation group. Further paired comparison showed that the difference between the vestibular rehabilitation group and the control group was of statistical significance (B=-3.88, χ2=18.29, P<0.01), while the difference between the betahistine group and the control group was not statistically significant (B=-0.96, χ2=1.16, P=0.28). The balance function of the three groups showed a trend of recovery over time, with no significant differences between groups (χ2=1.37, df=2, P>0.05). The median duration of residual dizziness for both vestibular rehabilitation and betahistine groups was 14 days, while that of control group was 19 days, with no significant difference between three groups[Log-rank (Mantel-Cox) test; χ2=1.82, df=2, P=0.40]. Conclusion: Vestibular rehabilitation can significantly improve the daily activities and social participation function in BPPV patients with residual symptoms following successful CRP, but its effects on shortening the duration of residual symptoms and promoting the recovery of balance function remain uncertain.

ABSTRACT Objective: To review the effects of the hammock positioning on clinical parameters of preterm newborn infants (PTNB) admitted to the Neonatal Intensive Care Unit (NICU). Data sources: This was a systematic review performed by searching the Pubmed, Lilacs, SciELO and PEDro databases. Intervention studies in English, Portuguese and Spanish that evaluated the effects of hammock positioning on clinical parameters of PTNB admitted to the NICU were selected. Three search strategies were used: 1) hammock positioning OR patient positioning AND intensive care units AND infant, newborn; 2) hammock positioning OR patient positioning AND intensive care units; 3) hammock positioning OR patient positioning AND intensive care units, neonatal. There was no restriction on the year of publication of the articles. Methodological quality was assessed by the PEDro scale. Data synthesis: Among 597 articles, only six were included and 139 neonates with gestational ages between 26 and 37 weeks and an average gestational weight <2240g were analyzed. Four studies included patients without any associated pathology and most of them placed the PTNB supine in hammock positioning. The duration of the intervention ranged from 15 to 180 minutes and most applied it at just one moment. There was an improvement in heart rate (HR), respiratory rate (RR) and pain (3/4 studies), as well as gains in peripheral oxygen saturation (SpO2) (2/4 studies). Only one study reported worsening of SpO2 with the intervention. The methodological quality of the studies was classified as low. Conclusions: Although this review suggests improvement with hammock positioning in HR, RR and pain in PTNB, the low methodological quality makes the results inconsistent.


RESUMO Objetivo: Revisar os efeitos da rede de posicionamento nos parâmetros clínicos de recém-nascidos pré-termo (RNPT) admitidos em Unidade de Terapia Intensiva Neonatal (UTIN). Fontes de dados: Trata-se de uma revisão sistemática realizada na PubMed, na Literatura Latino-Americana e do Caribe em Ciências da Saúde (Lilacs), na Biblioteca Eletrônica Científica Online (SciELO) e na Base de Dados em Evidências em Fisioterapia (PEDro). Selecionaram-se estudos de intervenção, nos idiomas inglês, português e espanhol, que avaliaram os efeitos da rede de posicionamento sobre parâmetros clínicos de RNPT admitidos em UTIN. Foram utilizadas três estratégias de busca: hammock positioning OR patient positioning AND intensive care units AND infant, newborn; hammock positioning OR patient positioning AND intensive care units; e hammock positioning OR patient positioning AND intensive care units, neonatal. Não houve restrição quanto ao ano de publicação dos artigos. A qualidade metodológica foi avaliada pela escala PEDro. Síntese dos dados: De um total de 597 artigos, apenas seis foram incluídos. As amostras totalizaram 139 neonatos, com idade gestacional entre 26 e 37 semanas e média de peso gestacional <2.240 g. Quatro estudos incluíram pacientes sem nenhuma patologia associada, e a maior parte deles dispôs os RNPT em supino na rede de posicionamento. A duração da intervenção variou de 15 até 180 minutos, e a maioria aplicou essa intervenção em apenas um momento. Foi observada melhora na frequência cardíaca (FC), na frequência respiratória (FR) e na dor (3/4 estudos), além de ganhos na saturação periférica de oxigênio (SpO2) (2/4 estudos). Apenas um artigo relatou piora da SpO2 com a intervenção. A qualidade metodológica foi classificada como baixa. Conclusões: Embora a rede de posicionamento pareça causar melhora na FC, na FR e na dor em RNPT, a baixa qualidade metodológica torna inconsistentes os resultados.
